Hold Harmless Waiver, Assumption of Risk & Release of Liability

In consideration of being permitted to participate in the Christ Cathedral Church & Christ Cathedral Outreach Ministry Walking4Life Walkathon, I voluntarily assume all risks associated with my participation in this event.

I understand that participation in a walkathon involves certain inherent risks, including but not limited to falls, contact with other participants, weather conditions, traffic, park conditions, and other unforeseen hazards that may result in personal injury, illness, disability, or death.

I, on behalf of myself, my heirs, executors, administrators, personal representatives, and assigns, hereby fully waive, release, discharge, and hold harmless:

Prophetic Ministries Covenant Alliance, Inc., Christ Cathedral Church, Christ Cathedral Outreach Ministry, Pastor Sherman C. Allen, Church officers, Trustees, Employees, Volunteers, Event coordinators, Sponsors, Public agencies, and any other individuals or organizations affiliated with this event, from any and all claims, demands, actions, causes of action, liabilities, damages, costs, expenses, attorney's fees, losses, injuries, or death arising out of or related in any way to my participation in the Walking4Life Walkathon, whether caused by negligence or otherwise, to the fullest extent permitted by law.

I acknowledge that I knowingly and voluntarily assume all risks associated with participation in this event.

I further agree that this Waiver and Release shall be binding upon me, my heirs, executors, administrators, legal representatives, and assigns.

I certify that I am physically able to participate in this event and that I assume full responsibility for my own health and safety during participation.

I acknowledge that I have carefully read Hold Harmless Waiver, Assumption of Risk, and Release of Liability, fully understand its contents, understand that I am giving up substantial legal rights by signing it, and signing it freely and voluntarily without inducement or coercion.

PHOTO & MEDIA RELEASE

I grant Christ Cathedral Church and Christ Cathedral Outreach Ministry permission to photograph, videotape, or otherwise record me (or my child) during the Walking4Life Walkathon and to use such photographs or recordings in promotional materials, publications, social media, websites, and other media without compensation.
